Předběžná náplň práce
Er:YAG laser je nová metoda laserové preparace, která slibuje šetrnější a přesnější stomatologické ošetření. Jeho specifickou charakteristikou je velká absorbce paprsku ve vodě a malá ve tkáni bez vedlejšího termálního efektu. Proto je ošetření bezpečné a efektivní.
Výzkum si dává ve své teoreticko – poznávací části za cíl posoudit její výhody oproti ošetření ostatními metodami – klasickou preparací. Jako základní prvek bude hodnocen rozsah léze vznikající při ošetření kariezního ložiska in vitro i in vivo. Bude posuzována velikost kavity, její tvar z hlediska vzniku smear layer a retenčního povrchu ve vztahu k výplni i zubní pulpě. Na extrahovaných zubech budeme v elektronovém rastrovacím mikroskopu posuzovat tvorbu kontaktní a nekontaktní preparace se zaměřením na tvar a velikost preparované léze ve vztahu k moderním výplňovým materiálům - nízkovizkozní kompozita, skloionomerní cementy.
Za podpory IGS MZ ČR: 9991-4 je na klinice rozsáhlý výzkum, který se zabývá použití různých terapeutických metod v ošetřování hendikepovaných pacientů. Stomatologická ošetření se speciálním postupem jsou časově i finančně náročnější než běžné stomatologické ošetření. Proto klinická aplikace teoreticko-poznávací části bude vytvoření systému na podporu rozhodování při ošetřování těchto pacientů. Systém by měl sloužit k rychlejší orientaci a vytvoření určitého vzorce v ošetřování pacientů, kteří kvůli svému hendikepu mají nestandartní režim terapie. Studie hodnotící stav chrupu těchto pacientů uvádějí, že tento stav je signifikantně horší než u pacientů bez hendikepu.
Tato práce kvantitativně zhodnotí výhody ošetření Er:YAG laserem a vytvoří systém, který zpřehlední možnosti, které je nutné zvážit u hendikepovaných pacientů při jejich ošetřování.
Předběžná náplň práce v anglickém jazyce
Er: YAG laser is a new method of preparation that promises gentler and more accurate dental treatment. Its specific characteristics is large absorption of the beam in the water and a small absorption in the tissue without the thermal effect. Therefore the treatment is safe and effective.
The research has the aim in its theoretical - cognitive part to assess its advantages in comparison with treaditional treatment method - drill. As an essential element will be evaluation of caries lesion in vitro and in vivo. The size of the cavity will be judged on, the shape of developing a smear layer and retention surface in relation to the filling and the dental pulp.
The extracted teeth will be scannes in electronic microscope to assess the formation of contact and non-contact preparation, focusing on the shape and size of the prepared lesion in relation to modern filling materials - low viscosity composites.
Extensive research supported by the Ministry of Health (IGA : 9991-4) that focuses on the the use of various therapeutic methods in the treatment of special needs patients. Dental treatment of a special needs patients is expensive and time-consuming than conventional dental treatment. Therefore, clinical application of theoretical cognitive part will create a electronical support system for the treatment of these patients. The system should be used for faster orientation and create a formula to treat patients who have a handicap due to their non-standard mode of therapy.
This paper quantitatively evaluates the benefits of treatment with Er: YAG laser and creates a system that will provide the transparent options that should be considered for handicapped patients in their care.
