Předběžná náplň práce
Předběžnou náplní práce s názvem Vliv cerebrálního hypoxického poškození na kognitivní funkce a psychosociální faktory bylo popsat závěry výzkumného šetření zkoumajícího dopady posthypoxických změn na kognici a prožívání pacientů.
Hypoxicko-ischemické cerebrální poškození v akutní i chronické podobě způsobuje zánik nebo změnu fungování neuronů. Patofyziologie cerebrální hypoxie je multifaktoriální a zároveň multioborovou problematikou, proto je v dosavadních výzkumech klinický obraz pacientů, kteří prodělali hypoxii, nejednoznačný.
Cílem předkládané práce bylo charakterizovat úroveň kognitivního poškození a změn v prožívání pacientů s mírnou intermitentní cerebrální hypoxií a u pacientů s těžkou jednorázovou cerebrální hypoxií. Dílčí hypotézy předpokládaly, že u pacientů s cerebrálním hypoxickým poškozením je nižší kognitivní výkon a vyšší míra úzkostných a depresivních projevů.
Klinický soubor sestával ze dvou skupin pacientů - model akutní hypoxie reprezentovali pacienti po srdeční zástavě a model intermitentní nebo-li chornické formy hypoxie představovali pacienti s diagnózou obstrukční spánkové apnoe. Testová baterie byla vytvořena z následujících testů: National Adult Reading Test/Czech Reading Test (NART/CRT), Montrealský kognitivní test (MoCA), Beckova škála depresivity (II. vydání, BDI-II) a Škála zjevné úzkosti a úzkostnosti (STAI X-1, X-2).
Preliminary content of the study called Cognitive and psychosocial sequelae following hypoxic brain injury was to describe the results of the research examining the posthypoxic impact on cognition and psychosocial factors.
Hypoxic-ischemic brain injury in acute and in intermittet form leads to neuronal necrosis and to neuronal functioning changes. Because the pathophysiology of cerebral hypoxia is both multifactorial and multidisciplinary issue, according to previous research studies the unified clinical picture of hypoxia patients is missing.
The aim of the present study was to characterize cognitive impairment and psychosocial changes of the patients exposing to mild intermittent cerebral hypoxia and of the patients after severe one-time cerebral hypoxia. Partial hypotheses comparing healthy individuals to hypoxic patients predicted impaired cognitive performance and higher level of anxiety and depressive symptoms in group of hypoxic patients.
The clinical sample consisted of two research samples - patients after cardiac arrest who represented the model of acute hypoxia and patients with obstructive sleep apnea represented chronic/intermittent hypoxia. The test battery was built from the following tests: National Adult Reading test/Czech reading test (NART/CRT), Montreal Cognitive Assessment (MoCA), Beck depression inventory (second edition, BDI-II) and State trait anxiety inventory (STAI X-1, X-2). In the group of sleep apnea patients the treatment effect on cognitive function and psychosocial factors should be verified.
The aim of the study was to develop a platform to synthesize knowledge of particular issue.
