Předběžná náplň práce v anglickém jazyce
Hand osteoarthritis (OA) is the most common musculoskeletal disease leading to joint pain, functional limitation and socioeconomic burden. Erosive osteoarthritis (EOA) represents a radiographic subset of hand OA and has worse clinical outcome than non-erosive disease. Diagnosis of EOA is based on the presence of central erosions and collapse of the subchondral bone plate of interphalangeal joints. Whether EOA represents a separate disease entity with specific risk factors and pathogenic mechanisms than non-erosive hand OA is not yet fully elucidated.
Pathogenesis of hand OA is unknown, but is considered to be multifactorial leading to heterogeneity of OA phenotype. Furthermore, there are number of promising candidate biomarkers; however none can serve as surrogate marker to be diagnostic, prognostic or sufficiently discriminating to differentiate between patients with erosive and non-erosive hand OA. However, there is some evidence that dysregulated expression of miRNA may play a role as a risk factor for the development of OA. Therefore, it can be speculated that measurement of circulating miRNA may represent novel diagnostic and/or prognostic biomarker(s) for hand OA (and potentially also other types of OA). Further idea of this project is the fact that EOA is associated with increased local inflammation, which may alter the expression of miRNA and thus discriminate between erosive and non-erosive disease.
The purpose of the proposed project is
- to characterize specific risk factor(s) for the development of erosive hand OA
- to discriminate erosive and non-erosive hand OA using circulating biomarkers with particular attention to circulating miRNA
