Topic Characteristics

Building on previous research from the US and Europe this thesis should expand the knowledge about the differences in health care costs among the regions and districts of the Czech Republic and their possible causes and explanations.
In the U.S. the relevant research is mainly focused on the geographic variation in Medicare spending as it is the largest single payer in the United States, providing health insurance for 52 million elderly and disabled beneficiaries. This was first documented by John Wennberg et al. with publication of the Dartmouth Atlas of Health Care in 1996, who pointed out on significant differences in spending per beneficiary at both the state and regional levels. The research then focused mainly on the causes of the variation, most recently Gottlieb et al. (2010) and Reschovsky et al. (2013), who attempted to explain the variance by the differences in price level across regions, but found out that this helped to explain only part of the overall inequality. Furthermore Zhang et al.(2012) adjusted the results for beneficiary-level demographic characteristics, insurance status, and clinical characteristics, but concluded that significant part of the variance still remains unexplained.

In the Czech Republic, such research is very relevant as there is a general system of public health insurance (compared to the U.S.) and hence systemic differences in health care cost across regions might be of a great importance for the public policy makers, pointing out to inefficiencies or possible misallocation of financing etc.

Hypotheses

Based on previous research we believe to find out that there is significant variance among regions in the health care cost in the CR. We would like to explore this hypothesis in more detail concerning the prescribed medical aids and pharmaceuticals.
H1: The variation is higher in regions with lower number of responsible doctors (less control between doctors)
H2: The variation is driven large hospitals (unevenly distributed across regions)
H3: The variation is affected by socio-economic conditions in region (e.g. unemployment)

Methodology and Outline
In this thesis we will use data from the Ministry of Health of the Czech Republic, which provide very detailed information for the whole CR, for all public insurance companies, enabling an in depth analysis of the topic. The core of the work will be a regression model which will allow us, to test different hypotheses about the differences in cost and to evaluate whether this variance is explained by the demographic characteristics or the price level of the regions. Apart from this we will use basic comparative analysis to explain and evaluate certain issues.
