Microbiology, Immunology and General Infectology The goal of the course is to explain the relationships between individual reactivity of humans, pathogenic microorganisms and environment.The content of the course is general and clinical medical microbiology, immunology and general infectology. Immunology is taught in the winter semester, general infectology in the summer semester, microbiology in both semesters. The student will receive a partial credit for each subject. Their set will be the basis for granting credit for the entire course. Partial credit can be obtained after completing the prescribed part of course, ie in the winter semester they will be in microbiology and immunology, in the summer semester in microbiology and general infectology. At the end of the summer semester, students will receive a credit from the course on the basis of four partial credits. The purpose of the course from the point of view of a microbiologist is to teach students to understand the relationship between a pathogenic microorganism and its host in terms of adverse biological properties of the microorganism (pathogenicity, virulence) and in terms of defense of macroorganisms (nonspecific and specific immunity). The description of this relationship may be specified in the relevant nosological unit (infectious disease). Furthermore, the content of the subject is laboratory diagnostics, basics of treatment and prevention of infectious diseases. The course also teaches the principles of disease and the basic manifestations of diseases that have a non-infectious mechanism and immune system disorder is the basic or significant cause of the disease (autoimmune diseases, conditions of primary and secondary immunodeficiency and diseases caused by hypersensitivity). To facilitate teaching, clinical situations and case studies are used in some topics. In the summer semester, the course will include lectures on pathology and seminars on pharmacology. The exam is oral and includes microbiology, immunology and general infectology.

The goal of the course is to explain the relationships between  individual reactivity of humans, pathogenic microorganisms and environment. The basis is to understand  interactions between various microorganisms and individual defense mechanisms of human organism, to specify and quantify both players of interactions - detection of pathogens and their property and features of immune mechanisms, it also includes principles of autoimmune disorders, immunodeficiencies and allergy. This educational process on the microscopic and molecular level  is supplemented by the clinical approach - demonstration of clinical cases with origin of infection or diseases caused by immune disorders. Students are expected to combine knowledge of pathophysiology, microbiology, immunology and histopathology - rational clinical science will be born from that. Students will get knowledge of clinical picture of infectious and immunopathologic diseases, diagnostic approach and basics of therapeutic approach.

Lectures will give basic informations about topics, seminars are based on interactive knowledqe acquisition. Practicum is based on immediate practical training of laboratory or clinical diagnostics procedures. Practicum at the clinic will allow to demonstrate the clinical science in reality.

Information about “General Infectology” classes (part of the subject MII)

Lectures take place at the Faculty building A. Seminars and bedside practices (mandatory lessons) take place at the Department of Infectious Diseases, building No. 7, at the Bulovka University Hospital. The entrance to the building for staff and students is from the bridge on the 2nd floor, from the north side (from the hill with the forest). Then, please walk up to the 6th floor where the students´ area is located. Please, bring your own FFP2 respirators, stethoscopes, and hospital footwear; you will receive gowns at the department.

Please, follow carefully the time-table in Výuka and/or in the e-Calendar. The start time of bedside practices and seminars must be respected. It is necessary to consider sufficient time needed to travel to Bulovka Hospital, including 10 minutes walking from bus or tram stop. We recommend to arrive by public transport because the parking area is limited. Searching for a parking space cannot be an excuse for arriving late to the lesson.

The only but strict requirement for being granted with the credit is 80% attendance at seminars and bedside practices, i.e. attendance at minimum two days out of three days of classes at the department (seminar plus practice). We recommend a full attendance. Compensation for exceeded absence will be allowed with another study group during the semester. It is not possible to compensate the absence with extra individual classes or practices at the infectious diseases department. Substitution of lessons or transfers of students to other study groups will be possible only with the consent of Dr. Bartoš or Assoc. Prof. Džupová.

Credit will be given to the student upon completion of the mandatory attendance and will be written into SIS immediately after the course. Without credit from Infectology, credit for the MII subject will not be granted and it will not be possible to register for the MII exam.

Requirements to the exam -

The condition for obtaining credit is the fulfillment of all the requirements of the subjects Microbiology, Immunology and General Infectology. The basic condition is 80% active participation in practical exercises/seminars (Microbiology and General Infectology). For the Immunology, the condition is 100% participation in practical exercises and 67% participation in seminars (6 seminars in total, 2 absences are tolerated). Furthermore, a satisfactory result of the interim tests (microbiology – 3 interim tests, immunology – a case report presentation). Credit can be obtained after completion of the required subjects, i.e. microbiology and immunology in the winter semester, microbiology and general infectology in the summer semester.

The final exam from the module will take place in the summer exam period (June - September). The exam in the module is an oral composite of the module's subjects (microbiology, immunology, general infectology, pharmacology).

Exam conditions:

- The exam can be taken a maximum of 3 times

- The exam takes place in the summer semester (min. 1 term), possibly the correction term can be taken during the examination period of the winter semester

- Appointments are listed in SIS, the condition for registering an appointment is credit from the subject and completed module C2VL004 (Structure and function of the human body)

- After registering for the exam, the term is mandatory

- To successfully pass the exam, you must succeed in all 3 questions of the tested subjects, the ranges of questions are the same as the syllabus of the subject

 

Part Immunology:

·         Case presentation: A group of 3 students will elaborate a description of clinical case of immmune mediated disease. Based on the short description, students will select appropriate diagnostic procedures and tests. The teacher will provide feedback during seminars, test results and advice. The case description consists from symptoms descriptions, pathogenesis of the disease, appropriate tests, probable diagnosis a possible treatment. Complete cae is presented by the group of all three students during specific time for case presentation (Exam period), discussed by others and evaluated by teacher

13. CASE STUDY

An 18-year-old man fell on his knee while playing basketball. The knee was swollen and remained painful the next day, so he was taken to the local emergency department. Clear fluid was aspirated from the knee, and the physician prescribed symptomatic treatment. Two days later, the swelling returned, the pain increased, and erythema developed over the knee. Because the patient also felt systemically ill and had an oral temperature of 38.8°C, he returned to the emergency department. Aspiration of the knee yielded cloudy fluid, and cultures of the fluid and blood were positive for Staphylococcus aureus.

15. CASE STUDY

A 35-year-old man with a history of intravenous drug use entered the local health clinic with complaints of a dry, persistent cough; fever; malaise; and anorexia. Over the preceding 4 weeks, he had lost 15 pounds and experienced chills and sweats. A chest radiograph revealed patchy infiltrates throughout the lung fields. A 57-year-old man was hospitalized in New York with a 2-day history of severe, watery diarrhea. The illness had begun 1 day after his return from Ecuador. The patient with craniocerebral injury was long-term hospitalized in the ICU. The patient was repeatedly treated with antibiotics. Artificial lung ventilation had to be initiated because of deterioration of respiratory parameters. The patient was febrile, and purulent secretion was aspirated from endotracheal tube. In endotracheal aspirate, Pseudomonas aeruginosa was cultivated.

A 78-year-old man confined to a nursing home awoke with a severe headache and stiff neck. Because he had a high fever and signs of meningitis, the nursing home staff took him to a local emergency department. The CSF specimen was cloudy. Analysis revealed 400 white blood cells per mm3 (95% polymorphonuclear neutrophils), a protein concentration of 75 mg/dl, and a glucose concentration of 20 mg/dl. Small gram-negative rods were seen on Gram stain of the CSF, and cultures of CSF and blood were positive for Haemophilus influenzae.

22. CASE STUDY

23. CASE STUDY

A 73-year-old man was admitted to the hospital because of breathing difficulties, chest pain, chills, and fever of several days' duration. He had been well until 1 week before admission, when he noted the onset of a persistent headache and a productive cough. The patient smoked two packs of cigarettes a day for more than 50 years and drank a six-pack of beer daily; he also had a history of bronchitis. Physical examination results revealed an elderly man in severe respiratory distress with a temperature of 39 °C, pulse of 120 beats/minute, respiratory rate of 36 breaths/minute, and blood pressure of 145 / 95 mm Hg. Chest radiograph revealed an infiltrate in the middle and lower lobes of the right lung. The white blood cell count was 14,000 cells/mm3 (80% polymorphonuclear neutrophils). Gram stain of the sputum showed neutrophils but no bacteria, and routine bacterial cultures of sputum and blood were negative for organisms. Infection with Legionella pneumophila was suspected.

24. CASE STUDY

A 61-year-old woman with left-sided face pain came to the emergency department of a local hospital. She was unable to open her mouth because of facial muscle spasms and had been unable to eat for 4 days because of severe pain in her jaw. Her attending physician had noted trismus and risus sardonicus. The patient reported that 1 week before presentation, she had incurred a puncture wound to her toe while walking in her garden. She had cleaned the wound and removed small pieces of wood from it, but she had not sought medical attention. Although she had received tetanus immunizations as a child, she had not had a booster vaccination since she was 15 years old. The presumptive diagnosis of tetanus was made.

A slightly cloudy cerebrospinal fluid was obtained from a lumbar puncture with a glycorrhagia of 0.22 mmol/l, a proteinorahia of 9.5g/l and a leukocyte count of 560/µl (of which 90% were neutrophils). Microscopic examination of the cerebrospinal fluid revealed numerous trophozoites approximately 10-25 µm in size that moved rapidly and directionally by means of pseudopodia. The cytoplasm of the trophozoites contained abundant granular vacuoles. The amoeba was identified as Naegleria fowleri. Despite intensive therapy, the patient died 6 days after admission.

54. CASE STUDY

A 42-year-old laborer living in a dormitory comes to the skin clinic for several weeks of exanthema and severe itching that worsens at night. Roommates also have similar problems. A diagnosis of scabies was made based on clinical symptoms.

1.      Do we have the possibility to confirm the presence of scabies as the causative agent of the disease somehow diagnostically, or how?

2.      What is the therapy? What is the method of transmission? Is this infection common?

3.      Other fungal infections can also appear on the skin. Give examples and options for therapy?

 

55. CASE STUDY

A 27-year-old hemato-oncology patient came to the hospital with acute nonspecific symptoms of fatigue, mild productive cough, and fever with chills, without hemoptysis or chest pain. The patient is on immunosuppression after an allogeneic bone marrow transplant for ALL (acute lymphoblastic leukemia). X-ray examination revealed the presence of a thick-walled cavitary mass in the right lung apex with centrilobular nodules consistent with aspergilloma along with patchy milk-glass opacities surrounding the alveolar infiltrate, this is referred to as the "Halo sign" in invasive aspergillosis. Bronchoscopy with bronchoalveolar lavage revealed dark fluid with suspended black particles, and fluid analysis showed high titers of aspergillus. Microbiological culture showed Aspergillus fumigatus. Immunosuppression was stopped and antifungal therapy started. Unfortunately, the patient died 16 days after admission due to the progression of the condition.
