The subject "Clinical Pharmacy II or Advanced Clinical Pharmacy" (summer semester, 4th year) is mandatory elective subject that develops the knowledge acquired in the basic subject "Clinical Pharmacy" (ZS, 4th year). Together with the subject "Clinical Pharmaceutical Care", it is one of the main subjects counted towards fulfilling the obligations of the Pregraduate Specialization Branch in Clinical Pharmacy. The subject „Advanced Clinical Pharmacy“ is suitable not only for students interested in acquiring deeper knowledge and skills to embark on the path of clinical pharmacist education, but also for students who are interested in gaining insight into the diverse possibilities that clinical pharmacy and the work of a clinical pharmacist offer in postgraduate studies, even if they have not yet fully decided for the path of clinical pharmacists.
To complete the undergraduate Specialization branch in the field of Clinical Pharmacy, completion of this subject and the subject Clinical-Pharmaceutical Care is a prerequisite. However, students interested in gaining insight into advanced areas of clinical pharmacy can also enroll in the „Advanced Clinical Pharmacy“ subject, even if they ultimately decide not to complete the entire specialization branch. For completing the Specialization Branch of the Clinical Pharmacy, a diploma thesis in the field of clinical pharmacy is preferred, but diploma these in related fields are also recognized after consultation with guarantors. It is recommended to complete the specialization branch with the State Doctoral Examination in Clinical Pharmacy, which is already recognized by some clinical-pharmaceutical workplaces when selecting colleagues for the positions of clinical pharmacists in training. But again, the State Doctoral Examination is not a prerequisite for enrollment in the Advanced Clinical Pharmacy subject, if students want to try this path.
The Advanced Clinical Pharmacy subject introduces students to the professional content of clinical pharmaceutical care in various environments and in various subfields (in the form of lectures and by the solution of clinical case studies in seminars demonstrating the application of the discussed knowledge in clinical skills). In one of the theoretical lectures, this course also introduces students to the development and functioning of clinical pharmaceutical care in the Czech Republic and to the development and functioning of clinical pharmacy departments and workplaces in the Czech Republic. The course is always taught in the form of connected lectures with seminars in one time block (when clinical cases are solved, these are more demanding clinical cases than in the basic course Clinical Pharmacy). Students thus delve deeper into the first knowledge and skills of individualized pharmacotherapy in various groups of patients and in various fields of internal medicine that cooperate with the field of clinical pharmacy. Advanced Clinical Pharmacy is an important platform of basic knowledge for signing to and for completing the course Clinical Pharmaceutical Care, where students practice assessing appropriate and inappropriate interventions under the guidance of experienced clinical pharmacists and physicians in clinical departments of the University Hospital in Hradec Králové.
Theoretical lectures and practical seminars in the „Advanced Clinical Pharmacy“ course introduce students to the principles of assessing the benefit/risk of pharmacotherapy in individualized treatment for various groups of patients. Knowledge and skills are dedicated to the procedures for assessing the maximization of efficacy and minimization of risks of pharmacotherapy while taking into account all necessary clinical factors (or knowledge of which basic clincial examinations and tests to request for this assessment), they focus in general part on the steps of the clinical pharmacist's work in assessing the effective and safe setting of a drug regimen, on the assessment of drug-induced risks and toxicity, on the withdrawal syndrome, assessment of treatment ineffectiveness according to objective and subjective markers, on deeper interpretations of the main laboratory markers in relation to the effectiveness and safety of pharmacotherapy, on the knowledge of monitoring of main parameters of the safe and effective pharmacotherapy and various rational ways of eliminating drug complications.
In specialized lectures and seminars, students meet practicing clinical pharmacists from clinical pharmacy departments and workplaces from various regions of the Czech Republic. They learn about their work and the essential areas of interventions that they focus on in various patient populations and in various internal disciplines. Students thus gain a first insight into what specific interventions (in addition to general ones) are essential for the work of a clinical pharmacist in these disciplines, e.g. in oncology, geriatrics, pediatrics, cardiology, infectology, intensive care, etc. The prepared lectures are not only attractive with this first insight and very practically oriented, but they also allow students to establish valuable contacts with lecturers and learn about the possibilities of further cooperation with clinical pharmacy departments and workplaces, e.g. within the framework of optional student internships or in preparatory clinical pharmacist positions after completing undergraduate studies.
Schedules of the lecture and seminars is sent by the guarantor of the subject to students before academic semester by email. Lectures and seminars on a given topic are always linked in one time block and preparátory materials are sent by the course guarantor in pdf format to students before a specific lecture/seminar. The course is concluded with an oral colloquial exam, which consists by preparing students in groups to solve a given case study by guarantor of the course. The case study is sent in advance, students can prepare in detail at home for the solution of the patient case and for the discussion with the examiner during the oral colloquial exam. The case studies are usually solved by groups consisting of 2-3 students.
The group of students solving the patient case should demonstrate on this patient case the ability to: -assess the severity of the patient's clinical condition and be able to request clinical assessments and laboratory results necessary for a basic comprehensive assessment of the safety and efficacy of a drug regimen by a clinical pharmacist -demonstrate understanding of the steps of a basic clinical-pharmaceutical review of the efficacy and safety of a drug regimen and be able to use these steps when solving a clinical case -identify (by using information on all clinical findings and risk factors) potential and likely clinically significant drug complications (side effects, drug interactions, signs of toxicity etc.) or other drug-therapy risks (ineffectiveness, withdrawal syndrome, etc.) -be able to determine a clinical-pharmaceutical plan for solving the patient's case and be able to correctly prioritize the solution of identified drug problems -be able to propose rational ways for solving drug problems by a clinical pharmacist -be able to propose basic preventive measures to ensure the future effectiveness/safety of pharmacotherapy after its optimal setting up
The preparation of the solution of a more complex clinical case study corresponding to the level of "advanced clinical pharmacy within undergraduate education" takes place in groups of 2-3 members. Students prepare a presentation supplemented with evidence of the identified drug and therapeutic problems and the solution paths. All students from each group present and all are equally tested in their reactions to the examiner's questions, who evaluates the adequacy of the answers.

The teaching takes place in teaching blocks dedicated to specific topics in the field of clinical pharmacy. These blocks consist of both theoretical lectures (1-2 teaching hours, depending on the needs) and practical demonstrations of the use of the lectured knowledge in skills in solving specific clinical cases of patients (2-3 teaching hours). Practical exercises are interactively led by experienced clinical pharmacists from various clinical-pharmaceutical workplaces in the Czech Republic.
In specialized blocks (lectures and then seminars), students meet practicing clinical pharmacists from various departments and workplaces of clinical pharmacy. They get to know their work and the essential areas of interventions that they focus on in specific populations and in various internal fields. Students gain a first insight into what specific interventions (in addition to general ones) are essential for the work of a clinical pharmacist in these fields, e.g. in oncology, geriatrics, pediatrics, cardiology, infectology, in intensive care, etc. The prepared lectures are not only attractive with this first insight, but also very practically oriented.
Direct interaction with clinical pharmacists, usually heads of various departments and workplaces of clinical pharmacy in the Czech Republic, allows students to also establish valuable contacts with lecturers and to learn about the possibilities of further cooperation with clinical-pharmaceutical departments and workplaces, e.g. within the framework of optional student internships or in future possibilities of obtaining positions of clinical pharmacists in preparation in these departments/workplaces (after completing undergraduate education). To successfully complete the course, an oral colloquial exam is required, which is a presentation and discussion of a pre-defined group of 2-3 students on a similar case study in the presence of the examiner. The case study is sent to the group of students in advance and the students prepare the solution for the oral examination by doing the homework together. The presentation for solving a clinical case should include: 1/ presentation of the patient's case 2/ identified drug problems and their risk with regard to the setting of the pharmacotherapeutic regimen, the patient's clinical condition and other factors/clinical conditions interfering with the treatment + evidence of citations/evidence for key conclusions 3/ rational proposals for solving the problems + evidence of citations/evidence for key conclusions 4/ conclusion of the case study Particular emphasis is given to points 2/ and 3/, where each student must be successful in solution and answerig the questions to examiner by at least 75%.
A group of students prepares a clinical case together, divides the parts of the presentation for the oral presentation equally (all students in the group present the individual parts equally). The examiner questions all students on areas related to the case study and they are to show in their answers that they can respond to the questions relevantly. The difficulty of the clinical case study is chosen to correspond to the level of "advanced clinical pharmacy" within the framework of undergraduate studies.

The subject Clinical Pharmacy II or Advanced Clinical Pharmacy introduces students to the content and professional content of clinical pharmaceutical care in various settings and in various subspecialties, both in the form of lectures and by solving clinical case studies demonstrating the application of the knowledge discussed in clinical skills. In this course, students gain deeper knowledge and skills of individualized pharmacotherapy in specific patient groups (geriatrics, pediatrics, nephrology, cardiology, etc.) and thus gain insight into the work and interventions of clinical pharmacists in various fields of internal medicine that collaborate with the field of clinical pharmacy. The subject Advanced Clinical Pharmacy (a compulsory elective subject in the 4th year of the LS) follows on from the subject Clinical Pharmacy (a compulsory subject in the 4th year of the WS) and, together with the subject Clinical Pharmaceutical Care (in which students practice at the patient's bedside under the guidance of experienced clinical pharmacists and doctors), is one of the main subjects of the undergraduate Specialization Branch Clinical Pharmacy. In specialized lectures and seminars, students meet practicing clinical pharmacists from clinical pharmacy departments and workplaces from various regions of the Czech Republic, learn about their work and the essential areas of specific interventions. Students have the opportunity to establish valuable contacts with these clinical experts and find out about the possibilities of further cooperation with clinical pharmaceutical departments and workplaces in the Czech Republic after graduation.
Learning outcomes: During the course, the student strengthens more advanced knowledge and skills in solving clinical cases of patients and after completing the course is able to: -assess the severity of the patient's clinical condition and be able to request clinical assessments and laboratory results necessary for an overall comprehensive assessment of the safety and effectiveness of the drug regimen by a clinical pharmacist -is able to apply the basic steps of a comprehensive clinical-pharmaceutical review regarding the assessment of the effectiveness and safety of the drug regimen when the necessary clinical examinations/parameters/markers are available (or upon request), on a specific more advanced patient case. He/she is able to use these review steps in the correct sequence -is able to identify (in connection with all the main clinical findings and risk factors) the expected clinical significance of frequent drug complications (frequent adverse effects, drug interactions, toxicities) or other problems of pharmacotherapy (ineffectiveness, withdrawal syndromes, etc.) -can establish a clinical-pharmaceutical plan for solving drug problems by a clinical pharmacist and correctly prioritize the steps of solving a clinical case -can suggest rational ways (from many possible ways) to solve identified drug problems - can suggest the main procedures for monitoring/maintaining the optimal setting of the drug regimen and recommend compliance with the necessary clinical conditions -in the first basics, is oriented in the different requirements for the work of clinical pharmacists in specific internal fields (pediatrics, geriatrics, intensive medicine, nephrology, hepatology, cardiology, palliative care, etc.) - knows the history of the development of clinical-pharmaceutical care in the Czech Republic, the key areas of activity of clinical pharmacists and the possibilities of future application in clinical-pharmaceutical services in the Czech Republic Last update: Fialová Daniela, doc. To enroll in the course "Clinical Pharmacy II or Advanced Clinical Pharmacy", a student should have completed the subjects Pharmacology and Clinical Pharmacy. If the student has completed lectures and seminars in these subjects and completed the subjects with the credit, but not with an exam, he/she can also enroll in the subject Clinical Pharmacy II/Advanced Clinical Pharmacy, but it is preferred that the student has at least passed exam in Pharmacology. If the student has not yet passed the exam in Pharmacology, he/she will discuss the possibility/suitability of enrolling in this subject with the course guarantor.
