Annotation -
The course Clinical Pharmacy I (CP I) focuses on fundamental knowledge of clinical pharmacy that is essential for: a) the follow-up course Clinical Pharmacy II (part of the Clinical Pharmacy undergraduate specialization, which students take as a mandatory course of this specialization). This course introduces students to the basic aspects of providing clinical-pharmaceutical care in clinical practice; and b) the follow-up course Pharmaceutical Care II (PC II), which trains students in applying fundamental knowledge regarding the clinical aspects of diseases and patient clinical problems when providing pharmacy-based pharmaceutical care. The core content of the subject Clinical Pharmacy I focuses on the etiopathogenesis and epidemiology of common diseases encountered in clinical practice; risk factors, including potential drug-related causes; clinical manifestations (symptoms, basic diagnostics); complications of untreated disease; therapeutic goals; and treatment strategies for various clinical situations (non-pharmacological measures and pharmacotherapeutic strategies, including indicated drug classes and specific agents). It also covers the review of drug properties (both class-specific and individual), fundamental knowledge of the risk-benefit profiles of drugs for major disease entities and common clinical scenarios, and absolute versus relative contraindications in clinical practice. The course develops the ability to assess drug indications and evaluate—in key aspects—the basic rationale behind the selection or adjustment of a drug regimen (at the CP I level, i.e., primarily regarding major indications and contraindications), taking into account patient comorbidities and standard population-based guidelines. The subject Clinical Pharmacy II (CP II) follows the Clinical Pharmacy I (CP I) subject within the undergraduate specialization. CPII deepens students' theoretical knowledge of clinical pharmacy, focusing particularly on the principles of individualizing drug regimens for specific population groups (e.g., patients with renal or hepatic failure, geriatric and pediatric patients, etc.). CP II also offers insight into the work of clinical pharmacists (specialists in the field) and provides a more detailed understanding of complex drug regimen adjustments based on the principles of individualized pharmacotherapy, emphasizing the practical application of knowledge gained in both CP I and CP II. Lecture materials (PPT presentations) are available to enrolled CP I students via Moodle: https://dl1.cuni.cz. Information regarding any changes to the lecture schedule is sent by the CPI course guarantor, Prof. Vlček — via the SIS platform or email. Study materials for seminar preparation or materials from completed seminars are distributed to the respective study groups by of English branch of the subject by prof. Vlček. Instruction consists of lectures and interactive seminars and requires significant individual study throughout the winter semester. Attendance at lectures is crucial, the content is updated annually to reflect the frequent changes in clinical guidelines. Textbook information is often outdated; therefore, we strongly recommend that students attend lectures and prepare for the exam using the current materials!!! Attendance at seminars is mandatory; missed seminars must be agreed with the guarantor of the English branch (prof. Vlček) which can require to work out the clinical study case on the relevant topic of the seminar. Before the examination period begins, in addition to obtaining course credit, students are required to prepare a case study for the oral examination. Through these clinical case studies, students practice the knowledge gained in the CPI course and demonstrate their ability to apply fundamental theoretical concepts to the resolution of clinical cases. English branch guarantor: Prof. Jiří Vlček, CSc. (contact: vlcek@faf.cuni.cz) Whole CPI course guarantor (Czech, English branch): Assoc. Prof. PharmDr. Daniela Fialová, PhD. Course completion requirements -

Credit (condition to pass the credit and to have it registered in the SIS)

a/ Presence on seminars

-          70% of presence on seminars of the English branch is requested by prof. Vlček (guarant of the English branch). If not fulfilled, it will be necessary to prepare one additional clinical case and to present it to the teacher – missing 3hours = 1 clinical case

b/ Successful passing of a credit test

credit test contains: aims, risk factors of disease, risk factors of complication, basic diagnostic methods + strategy of disease management (pharmacological, non-pharmacological) discussed during the seminars

Topics to prepare for the credit:

Basic principles of rational pharmacotherapy

Arterial hypertension

Dyslipidemia, atherosclerosis

Ischemic heart disease

Congestive heart disease

Diabetes mellitus

Thyreopathy

Pain management

Rational pharmacotherapy of infectious disoders

Respiratory and urinary infections

GIT disorders (peptic ulcer, GERD, diarrhea, constipation)

Asthma bronchial, COPD

Anemia

Depressive disorders

c/ preparation of your own clinical case and discussion on it (usually ill relative, friend, etc.). Evaluated is also the knowledge of the mechanisms of action of medicines, their major indications and contraindications, what are the goals of the treatment and which of them are reachable and how. The same case (improved after the credit exam) will be discussed also  during the oral examination.

Oral exam – condition for passing the exam and registration in SIS

1/ only students with fulfilled prerequisites can register for oral examination in SIS

2/ passed credit from clinical pharmacy 1 is necessary condition

Consultation before exam are scheduled during the time or after the lectures and on January 2027 (as agreed between students and prof. Vlček).

If students need additional consultation, prof. Vlček is willing to offer them individually according to the needs of students, but without payment only for students participating on at least 70% of lectures! Max 1,5 hour of consultation is offered.

Oral exam

1/ Question No 1: Knowledge regarding selected nosological unit in this order:

Short characteristic of the disease

Etiopathogenesis

Causes and triggers of the disease

Complication of non-treated or wrongly treated disease

Epidemiology and risk factors of disease

Symptomatology

Diagnosis (the student is able to answer 5 questions: 1/ does the patient suffer from disease?; 2/ what is the severity of disease and are there present complications of a particular disease?; 3/ are there present risk factors and triggers of a particular disease? 4/ are there present risk factors of complications? 5/ does the treatment reaches the aim? Which diagnostic methods help us to answer these questions?

Aim of treatment

Strategy of the treatment of the acute and chronic form of disorders (focus mainly on pharmacotherapy, but the students should know also major non-pharmacological methods of the treatment)

Pharmacotherapeutic strategies of the treatment (therapeutic properties, pharmacology, evidence based risk/benefit of these methods, populational experience - these are additional information that can the discussed also to explain the role of pharmacotherapy);

2/ Question No 2: Main indications and contraindications of selected group of drugs, incl. theoretical explanations of mechanisms of therapeutic effect and side effects, class effects and individual properties of medications. Questions focusing on medicines used for the treatment of disorders stated above (taught during the lectures and seminars)

Not all topics for exam are going to be lectured, we believe that you can self-study from different sources and from the past lectures at the University also. During consultation, you can ask prof. Vlček if your study materials and way of studying is appropriate.

For Erasmus students without prerequisites for Clinical pharmacy the time of the exam will be enlarged for additional 30 minutes to test students pharmacologic knowledge necessary for selected topics of question 1 and 2. Self-study of pharmacological properties of medicines used in major disorders lectured is required, sometimes consultations at the Department of Pharmacology are necessary. Good knowledge from pharmacology is necessary condition for passing the subject Clinical pharmacy (students should show broader knowledge regarding basic pharmacodynamic and pharmacokinetic properties of medicines and knowledge of their class effect).

Learning outcomes -

The subject Clinical Pharmacy I is a main, integrating, clinical subject that connects clinical, epidemiological and pharmacological knowledge about diseases, the therapeutic process, the patient and drugs used and uses the basic principles of determining population and individual efficacy and safety of pharmacotherapy in individualized treatment. The subject builds on the knowledge and skills acquired by students in previous therapeutically oriented subjects: Human Morphology and Physiology, Immunology, Pathological Physiology for Pharmacists, Microbiology, Molecular Biology and Genetics, Toxicology for Pharmacists and Pharmacology.
The prerequisite for the subject Clinical Pharmacy I. is the completion of the subject Pharmacology. The subject Clinical Pharmacy I. is a prerequisite for the specialized branch Clinical Pharmacy, especially the subjects Clinical Pharmacy II and Clinical Pharmaceutical Care, which allow a deeper look into clinical pharmaceutical knowledge and skills needed on the path of growth towards the future profession of a clinical pharmacist. The subject Clinical Pharmacy I is also a prerequisite for the subject Pharmaceutical Care II, which trains basic knowledge and skills in the field of medication counseling, focused primarily on the effective and safe use of medications and medical devices by patients, support of patients for rational self-medication treatment, and rational implementation of home monitoring.

Learning outcomes

Based on the acquired knowledge, the student will be able to:
Ø define the importance of clinical pharmacy as a field and the need for clinical-pharmaceutical care in various healthcare settings
Ø describe the levels of education in clinical pharmacy and professional organizations supporting the development of clinical pharmacy
Ø explain the epidemiology of the occurrence of selected basic diseases, their etiopathogenesis, main risk factors (non-drug and drug-related, controllable and uncontrollable), basic diagnostic methods (symptoms, most frequently used diagnostic tests, methods), subtypes and stages of the disease, basic complications of the disease and will be able to use this knowledge in assessing the patient's risk and determining the basic goals of therapy in clinical case reports
Ø determine the basic goals of therapy and especially pharmacotherapy, namely populational (for nosological units, or their subtypes) and individual (based on the patient's clinical case, in basic aspects)
Ø explain the main strategies for treating the most common diseases (strategies for treating acute and chronic forms, with an emphasis on basic pharmacotherapeutic strategies and basic knowledge of the main non-pharmacological procedures)
Ø describe the basic indicated drugs/drug combinations recommended according to the EBM ("evidence based medicine") in various stages of chronic diseases and in acute situations, also for the treatment of complications of the disease
Ø estimate the basic expected efficacy and basic risks of pharmacotherapy in a specific clinical situation, using pharmacological knowledge (about the mechanisms of desirable and undesirable effects of drug groups and drugs, pharmacokinetics and pharmacodynamics of drugs), and pharmacoepidemiological and clinical knowledge
Ø evaluate and interpret basic available data about the patient, including very basic laboratory results and clinical data, estimate the need for additional information (clinical and other) for a relevant assessment of the basic efficacy and safety of pharmacotherapy
Ø assess the appropriateness/inappropriateness of pharmacotherapy at the level of basic indications, contraindications, possible main adverse effects and basic drug interactions (knowledge of dosing is not required at this stage!)
Ø suggest the main procedures for monitoring of achieving therapeutic goals, for monitoring complications, control of risk factors, as well as for monitoring the expected main benefits/main risks of pharmacotherapy.

Øis able to estimate at the basic level the probability of achieving/not achieving the therapeutic/pharmacotherapeutic goals
