Annotation -

The education of medical genetics in the 5th year consists of eight seminars and one attendance on the Department of Medical Genetics. Teaching is based on the revision of knowledge from the 1st year of the study and on the introduction of details of particular topics. The seminars are focused on clinical cases and casuistics. The visit on the Department of Medical Genetics demonstrates the work in the genetic counselling centre, conjunction of genetics with other medical specialities, work in cytogenetic and molecular genetic laboratories.

Aim of the course -

The lessons from medical genetics - course "Clinical Genetics" in the 5th year of the study run as a part of the Pediatry module. They are consequential with the genetic lessons in the 1st year within the Cellular basic of Medicine modul. The aim is bring new informations about diagnostic possibilities of hereditary diseases, provide students with the necessary knowledge in the field of medical genetics and due to give them the ability to indicate a genetic consultation and methods no matter their specialities.

Znalosti

Choroby monogenní a polygenní, autozomálně dominantní a recesivní choroby, gonozomálně dominantní a recesivní choroby, strukturní chromozomální aberace, aneuploidie, mozaika, mutace somatické, penetrance, expresivita, symptom, syndrom, fenotypová heterogenita, nosič choroby.

Všeobecný přehled o vrozených vývojových vadách (vvv), znalost jejich etiopatogeneze.

Kardiogenetika: kardiomyopatie, arytmogenní syndromy, onemocnění aorty + chlopenní vady + vvv.

Syndromy spojené s dynamickými mutacemi a s poruchou imprintingu, mikrodeleční syndromy.

Genetika nádorů: sporadické, hereditární a familiární malignity, multifaktoriální predispozice, preneoplastické syndromy, syndromy chromozomální instability, protoonkogeny, virové onkogeny, tumor supresorové geny, mutátorové geny, familiární retinoblastom, LOH, familiární nádor prsu a ovaria, kolorektální karcinom, Li-Fraumeniho syndrom, chromozomální abnormality, které jsou příčinou a důsledkem nádorového onemocnění.

Dovednosti

Principy genetického poradenství, znalost genealogie, rodokmenu a genetických rizik.

Indikace k vyšetření dysmorfologickému, cytogenetickému a molekulárně genetickému, nejnovější metody genetického vyšetření, jejich indikace a omezení.

Základní postupy lékařské péče o těhotné ženy, medicínská rizika během těhotenství a speciální péče o nemocné těhotné ženy, přistup k pacientkám způsobem vedoucím k prevenci a rozpoznání jednotlivých vrozených vývojových vad.

Diagnostika a genetická prognóza mikrodelečních syndromů a jejich fenotypové variability.

Indikace ke genetické konzultaci u nádorových onemocnění, specifika v rodinné anamnéze, možnosti jejich prevence, prediktivního testování, a molekulárně genetických vyšetření nemocných a osob v riziku.

Postoje

Student bude znát principy dědičnosti genetických chorob, metody genetického poradenství v diagnostice genetických chorob, získá přehled o fenotypových důsledcích genetických chorob a možnostech prevence.

Student bude chápat úlohu cytogenetiky v genetickém poradenství, bude znát indikace k chromozomálnímu vyšetření postnatálnímu a prenatálnímu, bude rozumět chromozomálním nálezům, které bude umět pacientovi vysvětlit.

Student bude chápat úlohu genetických vyšetření u malignit pro diagnostiku, subklasifikaci, prognózu, racionální výběr terapie a monitorování léčebného účinku.

Requirements to the exam -

Requirements for credit:

- fulfilment of eight credits (8 seminaries or 8 seminar works, and all can be arbitrarily combined)

- attendance of the Dept. of Medical Genetics in Thomayer Hospital (if the week before the practical training will not stay logged on one, it will not take place)

- passing the test at minimum 60 points (terms of tests are provided in the schedule), consultation and looking for texts take place in one week after the test.
