Preliminary scope of work
Práce se bude zabývat problematikou nutriční intervence a rolí nutričního terapeuta v procesu léčby u pacientů s kritickými termickými úrazy. V práci budou popsány standardizované postupy nutriční podpory. Součástí práce bude sledování dvou skupin pacientů s rozsahy popálenin II. – III. stupně na 15 – 40 % tělesného povrchu. U těchto pacientů bude substituován selen v doporučených dávkách dle ESPEN Guidelines. V jedné skupině pacientů bude forma aplikace parenterální, ve druhé perorální. U pacientů budou monitorovány hladiny selenu v krvi. Tyto hladiny budou korelovány s klinickými parametry, jako je délka hospitalizace v režimu intenzívní péče. Cílem práce je zhodnotit efektivitu nutriční péče a dynamiku sérových hladin selenu při stejném dávkování, ale rozdílné cestě podání.
Preliminary scope of work in English
The thesis will focus on nutritional intervention and role of clinical dietitian during therapy of critical thermal injury. Standard therapeutic procedures will be clearly described. Clinical section of the thesis will observe two groups of patients with II-III. grade burns on 15-40% of body surface. These patients will receive selenium supplementation in doses corresponding to ESPEN Guidelines. One group will receive selenium parenterally, the other enterally. Plasma selenium levels will be monitored and subsequently correlated with clinical parameters, such as length of stay in intensive care. Aim of the thesis is analyzing effectivity of nutritional support and plasma selenium dynamics during different modes of administration.
