Preliminary scope of work

Bakalářská práce se zabývá tématem reverzních endoprotéz ramenního kloubu a funkčními výsledky ramenního kloubu po jejich implantaci.
Cílem práce je aplikovat popsaný fyzioterapeutický přístup na vybraný soubor pacientů a následně zhodnotit výsledky funkce ramenního kloubu v daných intervalech v průběhu pooperační fyzioterapeutické intervence. Cílem práce je také analýza současných studií na toto téma, porovnání s jejich výsledky a porovnání efektivity různých fyzioterapeutických přístupů.
Teoretická část shrnuje poznatky o anatomii, kineziologii ramenního pletence a biomechanice ramenního kloubu. Dále se věnuje endoprotézám ramenního kloubu, především detailně popisuje reverzní implantáty. Následně shrnuje zásady fyzioterapeutické pooperační intervence a nastíní různé přístupy fyzioterapie.
Praktická část je zpracována formou výzkumu, jehož soubor obsahuje 8 pacientů, kteří podstoupili implantaci reverzní endoprotézy a následnou fyzioterapeutickou intervenci. Pro klinické zhodnocení funkce ramenního kloubu byly zvoleny tyto parametry – aktivní a pasivní rozsah pohybu, Constant shoulder score, Shoulder pain and disability index a bolest dle numerické škály bolesti při provádění denních aktivit. Měření probíhalo v intervalech 4-6 týdnů, 3 měsíce a 6 měsíců od operace.
Závěrem práce je, že fyzioterapie má velký význam v pooperačním období. Výsledky potvrdily cíle práce, tedy významné zlepšení všech měřených parametrů. Aktivní fyzioterapeutický přístup by měl být indikován pro svoji bezpečnost a velmi uspokojivé klinické a funkční výsledky.

Preliminary scope of work in English

This bachelor thesis deals with the topic of reverse shoulder arthroplasties and functional outcomes of the shoulder joint after the implantation.
The aim of this thesis is to apply described physiotherapeutic approach to a selected group of patients and to evaluate the outcomes of shoulder joint function at given intervals during the postoperative physiotherapeutic intervention. The aim of the thesis is also to analyse current studies about this topic, compare the results and compare the effectiveness of different physiotherapeutic approaches.
The theoretical part summarizes knowledge about the anatomy, kinesiology of the shoulder girdle and biomechanics of the shoulder joint. It also discusses shoulder replacements, especially the reverse implants. Furthremore, principles of physiotherapeutic postoperative intervention are described and physiotherapeutic approaches are outlined.
The practical part of the thesis is procesed in the form of a research. It includes 8 patients who underwent implantation of a reverse arthroplasty and subsequent physiotherapeutic intervention. For clinical evaluation of shoulder joint function these parameters were chosen – active and passive range of motion, Constant shoulder score, Shoulder pain and disability index and pain according to numeric rating scale during daily activities. The measurements were carried out at 4-6 weeks, 3 months and 6 months after surgery.
The conclusion of the thesis is that physiotherapy has great importance in postoperative care. The results confirmed aims of the reserach, that is significant improvement in all measured parameters. An active physiotherapeutic approach has very satisfactory clinical and functional outcomes and should be indicated.
