Preliminary scope of work

V této diplomové práci se zabýváme ventilátorovou pneumonii (VAP) jako jednou z obávaných nozokomiálních nákaz na ICU a možnostmi prevence vzniku VAP při poskytování ošetřovatelské péče pacientům na odděleních tohoto typu. V teoretické části práce jsou začleněny kapitoly o nozokomiálních nákazách s širším zaměřením na VAP, její patogenezi a důsledcích umělé plicní ventilace spolu s uměle zajištěnými dýchacími cestami endotracheální intubací nebo tracheostomií. Pozornost je věnována především ošetřovatelské péči o pacienta na umělé plicní ventilaci v základních oblastech jako je bezpečnost a komfort nemocného a péči o dýchací cesty a ventilační okruh. Hlavní úlohou ošetřovatelské péče je především prevence. Proto se v práci zabýváme také ošetřovatelskou péči orientovanou na preventivní opatření VAP, hlavně na péči o dutinu ústní a dýchací cesty, polohování nemocného a enterální výživu.
Cílem empirické části práce je zjistit, jakým způsobem je v praxi realizována prevence VAP při poskytovaní ošetřovatelské péče s ohledem na doporučené postupy v jednotlivých zdravotnických zařízeních zapojených do výzkumu.

Cíl práce: Zjistit, jakými způsoby je v praxi realizována prevence VAP při poskytování ošetřovatelské péče.
Dílčí cíle:1. Zmapovat preventivní opatření na vybraných klinických pracovištích;
2. Porovnat preventivní opatření mezi jednotlivými pracovišti;
3. Porovnat preventivní opatření jednotlivých pracovišť s ohledem na doporučené postupy;
4. Zjistit informovanost sester o VAP a preventivních opatřeních bránicích vzniku VAP.

Preliminary scope of work in English

In the following thesis we have focused on ventilator-associated pneumonia (VAP) as one of the dreaded nosocomial infections seen on ICU trying to determine the possibilities of prevention of VAP during nursing care. In the theoretical part of the work we have included chapters on nosocomial infections emphasizing on VAP and its pathogenesis as well as the consequences of artificial lung ventilation through endotracheal intubation or tracheostomy. We have focused on nursing care of the ventilated patient in its main key areas, which are patient’s safety and comfort and care of his airway and ventilation circuit. The main goal of nursing care is primarily prevention. Therefore in our thesis we also discuss nursing care issues oriented on prevention of VAP, mainly through the oral cavity and respiratory care, positioning of the patient and enteral nutrition.
The goal of the empirical part of our thesis was to determine the implementation of VAP prevention issues in practice while taking into account the guidelines in the various health institutions involved in the research.
Objective: To determine the practical arrangements of VAP prevention during nursing care.
Sub-objectives:
1. To determine existing preventive measures at chosen clinical facilities;
2. To compare the measures between different facilities;
3. To compare preventive measures of the facilities regarding their guidelines;
4. To determine awareness concerning VAP and its prevention among the nurses.
