Stanovování cílů je nedílnou součástí rehabilitace. Cíle mohou přispět k využití přístupu zaměřeného na člověka. To může pomoct ke kvalitnějšímu poskytování zdravotní péče, ale také k motivaci člověka a lepším výsledkům v rehabilitaci pacienta. Diplomová práce se proto zabývá stanovováním a plánováním cílů v neurorehabilitaci. Hlavním cílem diplomové práce je zmapování nástrojů a metod pro stanovování cílů v neurorehabilitaci. Praktická část diplomové práce má za cíl zmapovat, jakým způsobem se nejčastěji stanovují cíle v neurorehabilitačních zařízeních napříč Českou republikou. V teoretické části je s využitím zejména zahraniční literatury vytvořen výčet nejčastěji využívaných nástrojů a metod pro stanovování cílů v rehabilitaci. Dále se zde nachází informace ohledně stanovování cílů u osob s afázií nebo o možnostech motivace pacienta k terapii. Praktická část diplomové práce byla tvořena online dotazníkovým šetřením, které bylo strukturované dle výsledků rešerše. Díky dotazníkovému šetření se získala data, která vedla k informacím o tom, že se v České republice pro stanovení cílů pacienta nejčastěji využívá metoda SMART, nebo se vychází z úvodního rozhovoru mezi pacientem a terapeutem. Stanovování cílů se většinou pacienti účastní, avšak při stanovování plánů terapie se pacienti zapojují o něco méně. Dochází shodě, že stanovování cílů je důležitou součástí rehabilitace a je potřeba, aby byly terapie vedené směrem k dosažení onoho cíle. |

Goal setting is an integral part of rehabilitation. Goals can contribute to the use of a person-centred approach. This can help to improve the quality of healthcare delivery, but also to motivate the person and improve patient outcomes in rehabilitation. This thesis therefore deals with goal setting and planning in neurorehabilitation. The main aim of the thesis is to map out the tools and methods for goal setting in neurorehabilitation. The practical part of the thesis aims to map how goals are most often set in neurorehabilitation facilities across the Czech Republic. In the theoretical part, a list of the most commonly used tools and methods for goal setting in rehabilitation is made using mainly foreign literature. Furthermore, there is information on goal setting for people with aphasia or on the possibilities of motivating the patient to therapy. The practical part of the thesis consisted of an online questionnaire survey that was structured according to the results of the research. As a result of the questionnaire survey, data were obtained which led to conclusion that in the Czech Republic the SMART method is most often used for setting patient goals, or it is based on an initial conversation between patient and therapist. Patients are usually involved in goal setting, but patients are slightly less involved in setting treatment plans. There is consensus that goal setting is an important part of rehabilitation and that therapies need to be guided towards achieving that goal. |
