Předběžná náplň práce

Pacienti s revmatoidní artritidou (RA) či osteoartrózou (OA) rukou mívají zhoršenou jemnou motoriku. Tu mohou ergoterapeuti objektivně hodnotit standardizovanými testy. Příkladem časově nenáročného a cenově dostupného testu je Devítikolíkový test (Nine Hole Peg Test, NHPT). Provádí se dle manuálu, který zahrnuje přesné slovní instrukce. Úkolem testovaného je co nejrychleji vložit devět kolíků ze zásobníku do otvorů v testovací desce a ihned je vrátit zpět. Stopuje se čas.
Cílem bakalářské práce bylo provést pilotní testování využitelnosti nového českého překladu manuálu Devítikolíkového testu na pacientech s revmatoidní artritidou nebo s osteoartrózou rukou.
Dílčím cílem bylo pořídit videodokumentaci během administrace tohoto testu a vytvořit doporučení pro využití Devítikolíkového testu v praxi u osob s RA nebo s OA.
Se souhlasem Etické komise Revmatologického ústavu bylo otestováno 19 pacientů (8x RA, 11x OA; 14 žen, 5 mužů). Průměrný věk skupiny je 61 let. Z testování byla pořízena videodokumentace.
Analýzou dat bylo zjištěno, že instrukce byly pro administrátora i všechny testované srozumitelné. Nejasnost se vyskytla u pokynů k provádění testu, kdy má testovaný poslouchat a pozorovat ukázku. Z důvodu imperativu chtělo 74 % testovaných následovat instrukce. Bylo nutné přerušit četbu instrukcí a situaci vysvětlit. Druhá nejasnost byla nalezena ve formulaci pravidla k řešení situací vzniklých během testování.
Nový český manuál NHPT je využitelný v praxi. V diskuzi byla navržena drobná úprava jeho textu. Byl prokázán přínos videodokumentace k analýze výkonu testovaného. Z pilotního testování vyplývá, že je vhodné u pacientů s RA nebo s OA sledovat míru pociťované bolesti před provedením NHPT i po něm.

Předběžná náplň práce v anglickém jazyce

Occupational therapists can objectively assess manual dexterity in patients with rheumatoid arthritis (RA) or hand osteoarthritis (OA) using Nine Hole Peg Test (NHPT). It is performed according to a manual that includes precise instructions. The subject is required to place nine pegs from the container into the pegboard as quickly as possible and return them straight away. The time required to complete the test is measured with a stopwatch.
The aim of the bachelor thesis is to perform a pilot testing of usability of a new Czech translation of the NHPT manual in patients with RA or with OA.
The partial goal is to obtain video documentation during the administration of the Nine Hole Peg Test and to create recommendations for using this test in practice in patients with RA or with OA.
With an approval of the Ethics Committee of the Institute of Rheumatology Prague, 19 patients (8x RA, 11x OA; 14 female, 5 male) were tested using NHPT.
Analysis of the data revealed that the instructions were comprehensible for the administrator and all patients. Some ambiguity was found in the part of the instructions where the patient should only listen and observe the demonstration.
The new Czech NHPT manual can be used in practice. Clarity could be improved by editing the ambiguity. Video documentation was made during the administration of the NHPT. The pilot testing suggests that it is appropriate to monitor the level of pain experienced before and after NHPT in patients with RA or OA.
